A Personal Letter from Terry Real
Welcome and thanks for joining me here…
Our clients come into our therapy offices abroil from both the promise and the demands of sex.
It is either too much, too little, or not the right kind.
While sex is such a critical issue for any healthy relationship, our clients are often reluctant to bring up the subject.
As a therapist, we need to be fully prepared to have this discussion with them and to help them navigate the increasingly complicated waters in our culture today. Bringing up the topic of sex can be uncomfortable and intimidating to many clients…and yet it often holds the key to how to help them in their relationships.
Even after many years of practice as a therapist this can be a tricky area, getting into the nitty-gritty details of sex, passion, intimacy, new relationship configurations, and sexual preferences.
However, because you’re serious about helping your clients create real change in their lives, you know that you have to be able to tackle the erotic with grace, knowledge, and authority.
The truth is that sex used to be simpler… Relationships were mostly monogamous, officially anyway, and stability, or at least it’s appearance, was paramount.
Now there are so many more layers and so many different configurations that there’s no “one-size-fits-all” approach. So as the world changes (think #metoo, gender fluidity, polyamory), how do we as therapists help our clients with insight and without our own prejudices, biases, assumptions?
How do we answer questions like: “When does sex turn into an addiction rather than simply acting out? How do you help couples negotiate different sexual “set points,” “Is sexting cheating?”
We are moving into uncharted and difficult territory.
But you are not alone and there is a roadmap. This course is both practical and challenging.
Together we’ll look at our own conscious and unconscious assumptions, what gets triggered in us, and how that affects this work.
This course will draw upon skills you already have, giving you first a therapeutic map, and then a pragmatic, easy to grasp toolbox. This is a comprehensive review of all aspects of the Relational Life Therapy (RLT) approach to issues of sexuality and betrayal.
Plus you will get to hear from six experts working in the field of LOVE and SEX.
If you’re serious about helping couples change their lives, change their relationships, then I invite you to join me in this course.
Warmly,
Terry Real

A New Way to Work With Your Clients
Creating Breakthrough Results for Your Clients
Inside RLT we take a unique path to help clients with issues of love, sex and power that differs from the following:
1. Often it is assumed that fixing the relationship’s emotional issues will solve the sexual issues as well. But typically that’s just not the case…
2. Many therapists are taught to take a passive approach. Saying, “I’m sorry, that’s terrible, tell me about it,”; however, this is completely unhelpful in these situations. Instead, this work demands an explicit and active approach.
3. Many therapists hesitate or are uncomfortable bringing up issues around sex because they are afraid that their personal views on sex won’t match the clients. But there’s a way to deal with that…
4. many therapists overlook the opportunities inherent in recovery from sexual betrayal. Instead of simply surviving infidelity, we want couples to THRIVE.
For over 30 years we have been pioneering a different way to do therapy that is more direct and is grounded in getting results.
Who Will Benefit from this Therapy Training Program
- You are a therapist who works with couples. If we’re being honest, almost all couples experience sexual issues at some point in their relationship, and they’re going to need someone to talk about it with. And that someone is likely to be you, their therapist.
- You want to feel more comfortable with whatever walks into your office. This course offers a comprehensive review of all aspects of the RLT approach to issues of sexuality and betrayal so that you are equipped with everything you need to help your clients with these issues.
- You are curious about the new frontiers of marriage and infidelity. We as therapists have to help couples through questions like, “is infidelity different for men and women,” “can you cheat in an open marriage,” “is monogamy obsolete,” and “what is the future of sex and relationship going to look like?”
- You want to explore where you are as you do this work. We will look at our own history, biases, and comfort level to see how this affects the way we treat our clients.

“Terry helps reawaken couples who seem trapped in a long-term stalemate and allows them to move toward growth and fuller Selfhood.”
– Richard Schwartz, originator of Internal Family Systems

What’s Inside the Training Ciricullum…
MODULE 1: Emotional Intimacy and Sexual Passion: Friends or Foes?
What is it that is so difficult about sex? It can be a great resource for a couple and it can be a source of some of the worst pain that a human being can experience. Sex is big. Sex is powerful. Sex has the capacity to disrupt and also to heal, like few things on the planet. It is unruly, politically incorrect, and quite possibly fundamentally transforming as I write.
As couples therapists, many of us have been taught that as the couple’s emotional intimacy progresses sexual issues will take care of themselves. This is wishful thinking. This module invites participants into a new conversation: How do we keep emotional intimacy and sexual passion alive at the same time?
We will review the current split in the field of couples therapy. While some thought leaders stress adventure and individuation, others stress security and attachment. What does it take to keep both alive, and, in particular, what does it take to keep passion alive in these changing times?
Participants will learn:
- How and when to bring up the subject of sex
- How to help couples feel comfortable talking about intimate sexual details
- How to identify and articulate the sexual choreography- Stance/Stance/Dance in the sexual domain
- How to relate the sexual choreography to the couples characteristic relational choreography
- How to conceptualize the many current changes in gender and sex as they relate to the process of psychotherapy
MODULE 2: Sexless Marriages, Discrepancies in Appetite, & Treating Sexual Trauma
More couples are referred for low sexual desire than any other presenting problems. Why does it seem inevitable that sexual passion recedes as familiarity deepens? Are there secrets to keeping passion alive in a long term relationship? And what of couples who present with what they describe as irreconcilable sexual differences, saying “We just have a different thermostat?” How does one negotiate something as personal and spontaneous as one’s own sexuality? And is negotiation even the right term to use in this context?
One out of four women in the United States has been sexually molested. And uncounted numbers of boys and men have experienced this trauma as well. How do you have a passionate, full-bodied sexual experience after you’ve been violated? In this section, we will outline specific steps to take when dealing with a sexual trauma survivor. Remember, by definition, the trauma means that you are no longer in control. Giving control to the traumatized partner is an essential first step.
This module focuses on helping couples rediscover passion through fierce intimacy, and the reappropriation of one’s own sexual desires and their fulfillment.
Participants will learn:
- How to shift the narrative from an essentialist (he just is that way) to a systemic orientation (my actions have something to do with this)
- How to assess each partner’s relationship to sexuality: how they give, how they receive, how they assert, how they surrender
- How to help each partner articulate in one another’s presence their sexual blueprint, their fantasies, and how to help each partner communicate what really turns them on without fear or shame
- How to help either or both partners reclaim their own rights and capacities for pleasure – Sex is not a chore, and it should never be coerced
- How to help partners shift from sex as caretaking to sex as constructive selfishness
MODULE 3: Sexual Betrayal: Coming Back from Infidelity
Infidelity is ‘universally forbidden, yet universally practiced,’ as my colleague Esther Perel says. And over time, infidelity has changed along with the social, cultural, and historical milieu. The one thing that’s remained constant is the question, “how can a couple come back from this level of rupture and betrayal?” In RLT, we aren’t interested in helping a couple simply survive infidelity. We want to use this crisis as a springboard for true transformation, in each partner and the relationship itself.
Are women having more affairs? Can people in open marriages cheat? Do happy people cheat, and if so, why? Are there different kinds of affairs? Does sexting count as infidelity? These are among a few of the many questions that arise when thinking about the changing nature of infidelity and how a couple can come back from this betrayal.
In this module, we will look at the differences between the hurt and involved partner, including the built-in asymmetry, the challenges each face, and what each partner needs to heal.
Participants will learn:
- The changing nature of gender roles and how that’s affecting the changing nature of affairs
- How to work with the involved partner’s grandiosity; helping your client move into healthy remorse
- How to work with the involved partner’s shame and pain; differentiating between unhealthy shame and healthy remorse
- What to do when the hurt partner is grandiose (a clinical challenge)
- How to differentiate between describing a feeling and emoting it
- Why revenge is not allowed (shelving resentment to get the work done)
- How to work with “reassuring behaviors;” how transparent should transparent be and why
- How to reshape the narrative that each partner has about the infidelity and why
MODULE 4: Sexual Addiction, Love Avoidants, and Reclaiming Sexual Health
Is there even such a thing as sexual addiction? In truth, the field is split on this issue. What does it mean to be a sex addict? What is the difference between addiction and sexual compulsivity? Is addiction to porn sex addiction? How do we assess and treat our sexually addicted clients?
Not all love avoidants are sex addicts, but all sex addicts are love avoidants. How does love avoidance show up in the bedroom, as well as in the general relationship? When and how do you feedback to the couple the nature of love avoidance and trackback into the client’s childhood to articulate its origins in trauma?
Participants will learn:
- How to move the sex addict out of extremes and into a moderate, healthy sex life
- How to help the partner of the sex addict to be discerning, self-protective, and yet open at the same time
- How to rebuild trust
- How to move the partner of the sex addict into restorative healthy sexuality as well
MODULE 5: Where to From Here? Sex in the 21st Century
Open marriage, polyamory, kink, gender fluidity, straight men having sex with other men – where are we headed? Is monogamy obsolete? Is the thought of one partner for life too daunting in our narcissistic “you can have everything” age? Can we really control jealousy? Can you have a polyamorous relationship and still cheat? Or what to say to a lesbian patient who admits painfully that she is less attracted to her partner since his transition?
We as therapists are facing situations and dilemmas undreamed of a generation ago. Should we be fellow explorers with no opinions of our own? Should we be guides? But then where is our own map? Are there general principles for good sex across multiple contexts and variations?
Participants will learn:
- How to face our own insecurities in this new fluid world
- How to look at our own biases and values
- What to do when you are faced with a client involved in situations outside your own value system
- How to deal with a client who is sexually irresponsible or offensive
- How to identify and deal with sexual predators
- How to listen to and celebrate new forms of sexuality and intimacy – allowing each person to tell their own story in full and in safety.
